About the Course

The Singapore Hands-On Live Endoscopy Course (SHOLEC) will be held at the Advanced Surgery Training Centre (ASTC) at the National University Hospital, Singapore on 7th - 8th April 2017. This year, the course will comprise of 2 programmes; one for the doctors and the other for the nurses.

 
+ SHOLEC for Doctors

Introduction

The course is designed with the Gastroenterology and Surgical Endoscopy trainee in mind as well as a select group of GI Physicians and Surgeons, eager to learn and share knowledge relevant to the treatment of patients. Most importantly, it allows participants to do Hands-on endoscopic procedures on live animals and simulator models under close expert guidance.

Course Content

The course will comprise of interactive lectures, supervised and assisted Hands-on session.

Hands on session

There will be a wide array of Hands-on stations which will include hemostasis, polypectomy, EUS, ERCP, PEG tube placement, endoluminal stenting & dilatation and exposure to balloon enteroscopy as well.

For the Haemostasis station, participants will be exposed to injection techniques, mechanical clip placement methods, APC settings and its appropriate use, when and how to use coagulation-based probes and band ligation techniques. The participants will also have hands-on experience with the latest Haemospray device.

Another basic therapeutic station will include polypectomy and EMR techniques on a stomach. This will include simple injection-lift technique to the more complex piecemeal EMR of larger lesions.

The Endoscopic ultrasound station will involve a model for better understanding of anatomical interpretation and learning basic techniques of tissue acquisition. Participants will be exposed to an EUS linear scope.

With the ERCP model, this station gives a great opportunity for beginners to learn cannulation and stenting techniques on a simulator under the supervision of an ERCP expert who can advise on the tips and tricks of cannulation.

Again with a live-case model, participants will be taught the concepts of double balloon enteroscopy as well as given tips for a successful therapeutic enteroscopy.

A separate station will be set up for PEG insertion tubes and techniques and allow demonstration of the direct and pull through methods of PEG placement. Participants will be guided through the correct step-by-step method of PEG placement to minimize risk of adverse events related with this procedure.

 
+ SHOLEC for Nurses

Introduction

This course is designed for nurses who have basic endoscopic knowledge, eager to learn the skills in endoscopy nursing and share their knowledge. Most importantly, it allows participants to gain a Hands-on experience to endoscopy procedures. Apart from having the exposure to various endoscopy related equipment, as well as the use of simulator and training models under different close expert guidance.

Course Content

The course will comprise of 2 sections. An interactive lectures and a supervised and assisted Hands-on sessions, led by Senior Endoscopy-trained nurses.

Interactive Lectures

  • Management of GI haemostasis
  • Clinical nursing management for ERCP
  • Different ways of removing polyp
  • Infection control in endoscopy

Hands on session

The Hands-on session will focus on three main areas in Endoscopy. They are Haemostasis, ERCP and Metallic Stenting.

For Haemostasis station, participants will be exposed to new modality on use of Haemospray; its indications and appropriate method.

For Endoloop station, participants will be introduced to the reusable and disposable loop. The techniques to deploy and tighten the loop.

With the ERCP model, this station gives a great opportunity for beginners to learn techniques on wire and cannula exchanges. There will also be an opportunity to learn insertion of biliary stenting techniques on a simulator under the supervision of ERCP nurse experts who can also advise on ERCP tips and tricks.

For the Metallic Stent station, the focus will be on the role of the nurses assisting in the insertion of the metal stent. Participants will be introduced to the different types of metal stents available, how to determine the radiopaque markings and finally how to deploy the metal stents.

KHOO TECK PUAT Advanced Surgery Training Centre (ASTC)

The Khoo Teck Puat Advanced Surgery Training Centre (ASTC) housed at NUH was originally built to train surgeons and endoscopist from Southeast Asia, the Middle East and Europe. The ASTC has the most comprehensive and latest facilities and equipment.

The Surgery and Minimally Access Research and Training Laboratory (SMART Lab) is equipped with 10 advanced operating theatre workstations comprising of endoscopy systems with high–resolution HDTV imaging. There are live animal stations that allow trainees to accomplish their training goals.

A seminar hall with the best interactive technology in the field of medical conferences, from 3D virtual reality to multi-live surgery transmissions to videoconference using DVTS link on 100 Mbps to transmit high definition live surgery from our Integrated Minimally Invasive Operating Theatres and from across the world.

The ST Lab, short for Simulation and Training Laboratory, the ST Lab is able to train surgical residents, local and international fellows, medical students and endoscopist using virtual simulation in a risk-free environment.

There are 6 surgical virtual simulators and 10 Dry training box simulators for minimally invasive surgery where trainees hone their skills. Simulators available are :

  • GI Mentor – Provide training for upper and lower gastro-intestinal disease. The trainees can perform simple basic procedures like gastroscopy and colonoscopy up to the most advanced operative endoscopic procedures like treatment of bleeding, ERCP including stenting and stones clearance
  • PEG model – A specially designed PEG placement model to allow trainee's to learn correct principles and thereafter practice PEG placements under supervision.
